Dear Texas Primary Care Provider
I am Sarah Compton, a graduate student in the Family Nurse Practitioner program at Texas State University, conducting a pilot study to identify by Self-report the rate Texas primary care providers are implementing the 2019 American Diabetic Association guidelines for their Type 2 Diabetes Mellitus patients. These include exercise and depression assessments in patients with type 2 diabetes mellitus (T2DM). You are being asked to voluntarily participate because you are an adult primary care provider who possibly treats patients with T2DM.
Participation is voluntary. The survey will take approximately 5 minutes or less to complete. You must be at least 18 years old to take this survey and a currently licensed Primary Care Provider.
This study involves no foreseeable serious risk and only potential minimal risks. Providers may feel a little uncomfortable answering particular survey questions as they reflect on knowledge of ADA guidelines. We ask that you try to answer all questions; however, if there are any items that make you uncomfortable or that you would prefer to skip, please leave the answer blank. You many quit the study and or questionnaire at any time. Your responses are anonymous and confidential.
There are minimal immediate benefits to the participants in the study. Participants may gain personal understanding of their own knowledge of and frequency of use of the new 2019 Care Standards for their adult Type 2 Diabetic patients including depression assessment and inclusion of exercise in the adult ambulatory care patient treatment plan.
Reasonable efforts will be made to keep the personal information in your research record private and confidential. The survey is anonymous. Do not put your name on the survey. The members of the research team and the Texas State University Office of Research Compliance (ORC) may access the data. The ORC monitors research studies to protect the rights and welfare of research participants.
Your name will not be used in any written reports or publications which result from this research. Data will be kept for three years (per federal regulations) after the study is completed and then destroyed.
You will not receive any financial compensation for your participation.

This project 6812 was approved by the Texas State IRB on November 3, 2019.
