We are seeking a compassionate Nurse Practitioner/Physician Assistant to join Hill Country Mission for Health (HCMH), a nonprofit clinic providing primary care to underserved adults in our community. This is an opportunity to make a real difference by helping patients manage chronic conditions, navigate healthcare, and improve their overall health.

Essential Functions:

• Provide primary care, including assessment, diagnosis, and treatment of common chronic and acute health conditions
• Manage patient care using limited resources, including labs, medications, and referrals
• Refer patients for specialty care, mental health, women’s health, or emergency services as needed
• Educate patients and families to support informed health decisions
• Document care accurately in our electronic medical record system
• Participate in staff meetings, community outreach, and clinic events

What We’re Looking For:

• Current Texas license as a Nurse Practitioner, Physician Assistant, or Physician
• BLS certification required (ACLS preferred)
• Strong computer skills; EMR experience helpful
• Compassion, professionalism, and cultural sensitivity
• Ability to work independently in a small clinic setting

Preferred:

• Bilingual in English and Spanish
• Experience serving underserved or uninsured populations
