Description: Our client is currently seeking a Part Time Nurse Practitioner Our client is seeking Nurse Practitioners in Dallas/Tarrant Counties to perform health risk assessments through house calls. This ongoing contract job requires NP's to travel within a 90-minute drive within counties in any direction for 10/15 assessments a week. Scheduling is based on needs and NP’s availability, with a competitive pay rate per assessment and mileage reimbursement.The job entails conducting history and physicals at patient’s homes or in health facilities. Accurate and timely clinical documentation of acute, chronic, and long-term healthcare issues are required. The provider will educate patients and/or patient’s families, complete referrals to case management, nursing, pharmacy, etc., and participate in clinical meetings and compliance trainings.

The Nurse Practitioner must be Family Medicine board certified and have an active/unrestricted Texas NP license. Proficiency in MS and use of a personal car with adequate automobile insurance are required.
